Job Description

Catholic Charities of Eastern Virginia is searching for a Full-time qualified Case Manager to work out of our Chesapeake office. The primary job of a Guardianship Case Manger is to coordinate and provide services for guardianship and conservator clients as required. Maintaining client records, preparing legal reports for the Department of Social Services, assisting with legal reports for submission to Commissioner of Accounts, apply for applicable benefits for the client, budget and manage client funds. Guardianship related tasks are performed as dictated by the Code of Virginia.

The ideal candidate will be an effective problem solver, possess strong analytical and interpersonal skills, ability to manage and prioritize multiple tasks, great attention to detail and accuracy, be able to work independently, excellent written and oral communications skills coupled with a positive attitude.

• College degree preferred in human services, social work or nursing background preferred, or related experience.
• Two or more years’ experience with the elderly, chronically mentally ill or intellectually challenged, and case management coordination.
• Experience with Microsoft Office, email, the Internet, and electronic record keeping.
• Valid Virginia driver’s license.
• Consumer Credit information including credit reports are obtained in strict compliance with the Fair Credit Reporting Act, the Americans with Disabilities Act (ADA), anti-discrimination and privacy laws and all other applicable federal and state laws.

• Assists with preparation of legal reports for submission to Commissioner of Accounts.
• Attends department meetings and training sessions.
• Maintains direct, on-going client contact on an as needed basis.
• Performs guardianship related tasks as dictated by the Code of Virginia.
• Performs conservator related tasks as dictated by the Code of Virginia.
• Must be able to operate a motor vehicle to transport clients, visit clients, and run errands on behalf of clients.
• Manages emergencies while on call with the emergency cell phone.
• Prepares reports to document case status.

Salary & Benefits

• 38,000 – 40,000 per year
