Life Coach Services
The Life Coach Program connects adults, teens, and children with chronic conditions who use the emergency department as their medical home to a primary care physician. This is done by the life coach scheduling the appointment for the client, calling them to remind them of the upcoming appointment, and then following up with the client to make sure they made the appointment and that everything went well. The Life Coach will also help to connect clients to community resources as needed.
Currently Catholic Charities’ Life Coach Program is available to patients at Chesapeake Regional Medical Center.
For more information on the Life Coach program, please contact Vanessa Dunlap, Director of Health & Community Services at (757) 875-0060, ext. 1033.
In-Home Respite Care
Do you care for an older adult in your home? Need a break from the rigors of caregiving 24/7? Catholic Charities of Eastern Virginia can help.
A trained respite worker will stay in the home with your loved one, providing companionship and ensuring their safety. In-Home Respite Care provides the caregiver with an opportunity to run errands, visit the doctor, or relax knowing that your loved one is being well cared for while you are away.
In-Home Respite Care is offered at $11.00/hr. Either the care receiver or care giver must be 60 years of age or older to receive services; care receiver must be over 18 years of age. Catholic Charities of Eastern Virginia will perform a free in-home assessment, and will match a trained, background-checked respite worker with your family.
Respite services DO NOT include bathing, personal care, housekeeping or transportation.
Services are provided to residents of Chesapeake, Hampton, Newport News, Norfolk, Poquoson, Portsmouth, Suffolk, Virginia Beach and York County.
